Funeral Establishments

INITIAL APPLICATION:

  • Newly Built (or under construction) - please request application at least 6 weeks before scheduled opening.  The facility must be inspected before you can open for business.
  • Purchased - please request application prior to closing on the sale.  The facility must be inspected before you can begin operating the business under new ownership. A NEW LICENSE MUST BE ISSUED WHEN THE FUNERAL HOME OR CREMATORY HAS A NEW OWNER.

RENEWAL APPLICATION:

If you did not receive a renewal application, you can download one from our web page under the FORMS tab.  All licenses expire June 30 of the renewal year.
 

    Many of our documents require the use of Adobe Acrobat Reader to view and print.  Adobe Acrobat Reader is free for download by clicking the icon to the left.  To print any of the Acrobat documents, you need to click the printer icon on the Acrobat tool bar instead of the printer icon on the web browser tool bar.
