- Newly Built (or under construction) - please request application at least 6 weeks before scheduled opening. The facility must be inspected before you can open for business.
- Purchased - please request application prior to closing on the sale. The facility must be inspected before you can begin operating the business under new ownership. A NEW LICENSE MUST BE ISSUED WHEN THE FUNERAL HOME OR CREMATORY HAS A NEW OWNER.

If you did not receive a renewal application, you can download one from our web page under LICENSING, then FORMS tab. All licenses expire June 30 of the renewal year.
